Logic Model Illustration
Apply the logic model for different health programs or interventions including assessing effectiveness.
This assignment is part 5 of 7 for your public health plan, which will be due in Unit 7. Create a logic model illustrating your public health plan.
Your illustration should use a combination of colors and appropriate design elements (text boxes, arrows, etc.)
Include as much detail as possible. You may create additional space in your model (for example, a textbox with only one key point) to notate areas you will expand upon further as your project continues development.
Include the following components: Inputs, activities, outputs, outcomes, impacts, assumptions, and contextual factors.
Format: You may create this logic model in any program (PowerPoint, Word, Publisher). Your submission must be in a Word document. If you use a program like PowerPoint to create, you can save the file as an image (jpg file) and insert the image into your Word document. Remember, you will need to access the original file you created to make appropriate edits prior to submitting with your final project in Unit 7.
APA requirements: Include a title page and reference page, minimum of three references.
