Obesity Case Study
Mary is a 33-year old woman who entered a medical fitness facility 10 years earlier wanting to lose weight, feel better, improve her health, and have more energy. She had been overweight for 12 years and wanted to lose 100 lb (45 kg) as a long-term goal. Her attitude was optimistic. She went through periods of weight cycling in the program. Her beginning weight was 350 lb (159 kg). Over six months of exercise and nutritionally balanced caloric restriction, she lost 8 lb (3.6 kg). Within nine months she had lost an additional 4 lb (1.8 kg). At the one-year mark she had gained back to 350 lb. she continued to cycle between 337 and 370 lb (153 and 168 kg) for the next 10 years. During this time, her adherence to exercise and diet was not consistent. Finally, her health and inability to lose weight make her a candidate for the Roux-en-Y gastric bypass surgery.
Mary is single, lives in an apartment with her two cats, and works as a human resources assistant at a local car dealership. She hopes that through this exercise program she can lose weight, maintain weight loss, and improve her overall health.
At work, Mary sits throughout most of the day and usually eats lunch at her desk. She played volleyball as a child but lost interest in high school because she wasn’t competitive enough to play on her school’s team. She used to play on a sand volleyball team in a summer league after she graduated college but stopped participating years ago. She visits her grandma twice a week in a nursing home and takes walks with her around the complex. She has friends from college that belong to the local YMCA who take aerobics classes together and she’s been meaning to sign up but still hasn’t because she is worried she won’t be able to keep up. She has been asked to join them for a few charity 5K and 10K runs but again is too embarrassed to sign up. She wants to participate in aerobics classes and hopefully be able to run a 5K in the future.
Currently, Mary does not participate in a purposeful exercise program. Her weekly activities consist of walking with her grandma a few times a week for 15 minutes, but aside from that she does no other exercising.
When asked the question, “On a scale from 1 to 10 (1 being the worst and 10 being the best), how would you rate your overall weekly physical activity?” Mary answered with a “2.” When asked, “Why did you not rate yourself a “1”, she replied, “I walk with my grandma a few times a week.” Then, when asked, “What would you need to do to answer the question with a “3?” she replied, “If I were to start going to the gym or doing an aerobics class at least 2 times per week, I would consider myself as a “3.”
Mary’s physical activity goals are to find a consistent exercise routine and lose weight. She wants to find a regimen she enjoys but also one that helps her maintain weight loss.
