Good afternoon,
My name is Chelsey, I am 29 years old and reside in New Port Richey, Florida. I currently work as a certified registered dental hygienist and have been working for two years within the health care community. I am working towards my Bachelor’s Degree in Health Services Administration and will continue to advance my knowledge within the field of health care. The top three things that I have learned within the Health Services Administration program have been to understand that individual differences can play a role in the ability to manage and create strategies, the understanding that a full detailed plan of action is important in order to implement change, and the ability to research topics in order to have a full array of knowledge before implementing changes.
I have used these three topics within my everyday work life. For instance, in a dental practice there are different jobs that have different responsibilities and it is imperative that we all work together to achieve success. The task of having a multiple person office means that details need to be organized and concise that way the office will run smoothly. The three topics listed above, I also implement with patients that I have in my chair. One approach may not work for everyone in regards to educating them on their dental health so it takes research, a plan of action, and being open minded in order to find what works best for the patient. These three topics should be introduced in every working environment because it allows for the ability to achieve success as well as create the necessary approach that works the best for the individuals being treated.

1. Jennifer had a long day and was trying to get out of the office before 5:00 p.m. She had one more person to see. Dr. Adams had asked Jennifer to give Abdul, a young man recently diagnosed with schizophrenia, a prescription for a new medication. Jennifer had her coat on when she handed the prescription to Abdul in the waiting room. Abdul wanted to know what the medication was and why his prescription was being changed. “Will there be any side effects?” he asked Jennifer. She replied hurriedly, “Oh, no. Dr. Adams says just take this until he sees you next time.”
In this vignette, there are unethical actions taken. The first is that Jennifer did not give the patient the time needed to properly inform the patient of the new medication. Because it was the end her shift, she was wanting to leave soon and did not put the patient’s well-being first. A strategy Jennifer should implemented, is to inform. Dr. Adams that Abdul wanted a consultation about the new medication. A patient has a right to know about any side effects and risks for not following through with taking the medication (Summers, 2016). In addition, Jennifer wears a white lab coat. It does not say in this vignette, if Jennifer is a doctor or not. Assuming that she is not, since the patient is there to see Dr. Adams, the apparel could be misleading. Since the late 1800s, the white coats has been a symbol that stands for professional doctors (Mahoney, 2018). It promotes trust between a patient and the doctor (Mahoney, 2018). When other professionals are not doctors and wear a white coat, it can be misrepresenting. Patients are more likely to trust these individuals, even though the individual is not a qualified doctor. Therefore, it is unethical for an employee to wear a white coat and speak on the doctor’s behalf. A strategy that could have been applied here is to have only doctors wear white coats in the facility.

Jim is doing an intake with a man who claims he is depressed. He tells Jim that ever since his wife left, he has had trouble concentrating and waking up in the morning. He talks about how lonely it is at home, how much he misses his children, and how he is tempted to drink in the evenings. Jim nods. He understands. “Yes, my wife left last month too, “Jim tells the man. “I know just what you mean. I get to feeling like, well, like there isn’t as much meaning. I never knew the kids were so important to me, but I guess they were. On Saturdays, I used to do things with my son, and I will get him every other weekend but it’s not quite the same thing, is it?” “No, the man responds , “I was thinking ….Jim interrupts the man to say, “ Well, I do a lot of thinking too. And so on.
The first wrong thing that Jim does is to self-disclose. He also interrupts his client while he is talking about his feelings and interjects Jim’s own feelings. Jim makes the session about himself and his own personal issues and forgets that it is his job to help the client. Jim needs to talk to a counselor himself about his own issues and release the client to someone else that can help this client. Jim may be impaired and in need of removing himself until Jim works out his issues and is able to handle his job in his usual manner (Summers, 2016,p. 66). Jim enters a dual relationship when he self-disclosed and began crying on his client’s shoulder. That relationship became social worker and friend and not social worker and client as it should be. The NASW Code of Ethics also addresses the idea of excessive influence over clients (Reamer, 2003).

If I were to change something about myself it would be to understand how a marriage can fall apart and how to not let it affect a relationship in the future. Using the levels of the ecological model perhaps I can do so. According to our author, Nancy Summers, there are many interactions that help to shape who we are and how we interact with people and the environment around us (Summers, 2016).These interactions known as micro, mezzo/meso, and macro can at times overlap and influence each other. In the micro stage, biology and psychological factors are helping to shape us (Summers, 2016). For me, I have inherited from my parents a work ethic that makes me want to do my best at what I am working at. Having been married and divorced and therefore a single mother, this work ethic was essential in my motivation to succeed in obtaining employment and provide a stable home for my son. Having supportive parents was definitely a plus. When Summers talks about the Mezzo/Meso level she is referring to how a person reacts to the society around him/her. (Summers, 2016). I think of the family, friends, and groups of people who I work with and enjoy spending time with. These same people encourage me to keep on going even when an obstacle gets in my way. I remember the strain that I felt in my small community where I grew up because some people looked at single mothers differently from those that were married. With help, love, and support my family and friends helped me overcome the small town prejudices. It did make me feel differently about church people though. The macro level that Summers refers to is the social aspect of the ecology model (Summers, 2016). This is where we either fit into the mold or create one of our own. As far as small town gossip I think that the people of a town have the ability to accept that all people are not the same and their situation is not the same. A small group of people should not be able to affect their opinions on someone’s life. Having moved to Florida and successfully raised my son on my own I feel vindicated in some way that he and I turned out okay. If I want to change one thing about me though having worked successfully through a bad marriage is to be able to accept that maybe part of the problem was mind. Did I come from a better home than he did and how could I have helped him understand that family is important and not an interference in someone’s life? But, in the end he is still involved in his son’s life and that is the important thing. The model has helped me understand that our backgrounds, our family, and our interactions with people and the environment around us helps shape who we are. Hopefully, I can use that to my advantage when working with clients.

Being shy is something I wish I could change about myself. Although I have made significant headway over the years, there remain elements of the shyness I struggled with in childhood; this problem could be confronted by a combination of counseling and gaining confidence over time. In learning what causes it, the problem could be approached from a solution-oriented perspective, which could help to reduce associated anxiety.
Micro level – this could be helped by learning more about the problem and using self-reflection to gain a better understanding of what may have caused it and what has helped in the past. It could be hindered by isolating.
Meso level – this could be helped by seeking counseling to understand the root causes of this problem, and by confiding in friends who have overcome it or are currently seeking to overcome it. It could be hindered by co-workers and acquaintances who do not experience shyness and confuse it with unfriendliness or aloofness.
Macro level – this could be helped by changing societal attitudes toward social awkwardness or shyness (including the tendency recently of seeing shyness as a “difference” rather than a “flaw”); however, it could still be hindered by lingering societal pressures to conform to norms which still see shyness as problem which should and could be “just gotten over” in order to be successful in life.
In addressing any problem, the ecological model can be applied to see how the micro, meso, and macro levels ultimately affect it, and can contribute to either helping or hindering progress. Using this model can help us as professionals, when we seek to view all factors which are involved in a client’s or patient’s problems.
