The Hyatt Regency walkway collapse occurred at the Hyatt Regency Kansas City hotel, in the famously known Kansas City caused the loss of more than a hundred lives and injured more than two hundred people. Havens Steel Company, who was the contractor that was responsible and monitored the manufacturing of the rods, is the one who is ultimately responsible for the fatal design flaw.

Havens Steel refused to comply with the original plan because it required for the entire rod that was to be placed below the fourth floor to be excellently screw threaded, to ensure a certain hold of the screw on the nuts of the fourth-floor walkway (Pfrang, Edward, & Marshall, 1982).
