Writing a clear evidence-based practice (EBP) question using the PICO format is the first step in developing a change in practice. In this Discussion, you will explore the steps involved in EBP and develop a researchable EBP question in the PICO format that addresses a patient quality and safety issue.

In this Discussion you will identify three clinical practice problems in your current (or past) practice as a nurse that risk compromise to quality of care and patient safety. You will select one of these problems to clearly define the risk and ask a question using the PICO format. Asking the question and selecting the problem is the first step of the Impact Model that is discussed in the textbook. Evidence-based practice questions are asked and written in the PICO format and you will use this process as you develop an evidence-based practice project that will be completed in steps during the course.

Note Initial Post: A three-paragraph (at least 350 words) response. Be sure to use evidencefrom the readings and include in-text citations. Utilize essay-level writing practice and skills, including the use of transitional material and organizational frames. Avoid quotes; paraphraseto incorporate evidence into your own writing. A reference list is required. Use the most current evidence (usually ≤ 5-years-old.)

Students: Please keep in mind as you work on your PICO questions that the PICO question must match the PICO elements. For example, if the intervention is to use a skin risk assessment tool, educate staff, and daily follow up skin assessment the PICO question should not be, what evidence reduces pressure ulcers in hospitalized patients? The interventions are not stated in the PICO and this is an issue. Does the use of a skin risk assessment tool, education of staff, and daily follow up skin assessment reduce pressure ulcers in adult med/surg patients? is a much better question.

Be specific as to the patient population, simply stating patients, all patients, surgical patients, etc. does not fully describe the population. Adult patients on a medical unit describes the population better. Do not attempt to focus the paper on all patients in the hospital as this will prove to be too large. Focus on one unit, area, or clinical setting.

Also, the comparison or contrast is not to compare pre and post rates of falls, pressure ulcers, etc. That would be an evaluation strategy. The comparison is to be a different intervention or no intervention. If the intervention is, “to conduct hourly rounding to reduce falls”, the comparison or contrast to this intervention needs to be based on current practice/policy such as, “use of fall risk assessment tool and call bell in reach of the patient with verbal instructions to ring for assistance”. This compares two interventions.

Remember, the ultimate goal is to read the literature, evaluate the information related to the topic and interventions being done and compared and then to develop a recommended change in practice based on the research in the literature. If a recommendation is not made because both the intervention and comparison are found to be equal or have no significant difference in outcome, then the Week 5 paper criteria will not be met.
