1. What happened (you can be creative here…. maybe a forklift flipped and hurt an employee and damaged merchandise, perhaps a container wasn’t secured and fell to the bottom of the ocean from the ship, a truck carrying eels spilled its contents and caused a lot of injuries). Assume whatever happened was bad enough to make headlines.
2. What procedure or process wasn’t in place or wasn’t followed to allow this to happen?
3. How is your team going to stop this from ever happening again?
4. How did you handle the team member(s) responsible?
