There is also the assessment of any coughing or wheezing in the patient these are caused by gastric contents in the trachea.On this assessment, the doctor records if there is any heartburn or any coughing and wheezing. The next assessment is checking if there is any hoarseness which comes by getting irritated in the vocals. The doctor will also ask the patient what time of the day him or her often gets the hoariness. The doctor will record these if it’s in the morning most patients experience in the morning.
