  • Mortality – top three (3) causes of death
• Using the attached template, complete the following:
  • In approximately one paragraph, summarize how you researched this data, if the rates and causes surprised you, and what opportunities for improvement exist for your community.
  • List one measure being used to address each morbidity and mortality factor you identified.
  • Briefly describe the measure (who it serves, how it is funded, etc. Bullet points are acceptable.)
  • Indicate if the current measures are effective (Yes or No). You will answer this question by evaluating how morbidity and mortality data has trended over several years.
