You are missing the following information:
The following information must be submitted electronically with the CE provider initial application:
General information:
• Briefly describe the purpose of your organization and how long you have been in
business.
• Please identify the contact person. If that person is not a nurse, it should discuss how
the nurse’s input will be obtained in developing and evaluating the programs.
• Please provide information on funding sources.
Operational Policies and Procedures:
• Minimum faculty qualifications must be discussed.
o Please explain in further detail regarding minimum faculty qualifications to
health care providers.
• If a person leaves before a program is completed will you provide partial credit? If
there will be partial credit, please explain how that will be determined. If not
applicable, please specify that person must stay for the entire event to be awarded contact
hours.
• Describe record-keeping policies and who will sign the certificates for the participants.
• Describe how records will be safely stored.
• Who will be responsible for signing certificates and record maintenance?
Ongoing Evaluation:
• Submit an evaluation tool.
• Who will be conducting ongoing evaluation of course materials, content accuracy,
quality and relevancy prior to each education event?
• Explain how offerings will be revised and need for additional subjects determined.
• How will you effectively assure the courses remain relevant and evolve to meet trending
healthcare and societal needs?
• Please submit an evaluation tool that collects information on the effectiveness, efficiency,
etc. of the overall program.
Home Study/Online/Correspondence Courses:
Self-directed learning experiences, including but not limited to home study, computer programs,
internet or web-based courses, are required to evaluate learner knowledge at the completion of
the learning experience. Please submit your policy on self-directed learning experiences that
must include the following information.
• The evaluation must include a minimum of 10 questions. Please submit the post-test
evaluation for online courses (Must include a minimum of 10 questions).
• The learner must achieve a minimum score of 70% on the evaluation to receive the
contact hours.
• The evaluation must be graded by the provider.
• A copy of the evaluation must be submitted with the application if submitting a self-directed learning course as part of an initial application.
• In courses or programs where participants learn and progress at their own pace, the
sponsor should establish a standard number of contact hours based on an average
number of hours required of several representative learners to complete the program.

determine the number of contact hours to be awarded for self-directed learning.
• One contact hour per 12,000 words of content, which is based on the average reading
speed of adults.
• List the procedure for issuing an attendance certificate to participants.
• Identify the length of time the participant must complete the course. Please identify the
length of time the participant must complete the course online/home study course.
• If you are planning on offering a Home Study/Online/correspondence course you must
submit a copy of one of these courses with your initial application.

Submitted offerings cannot be a mandatory CE program (submission of courses is not required if you are accredited by the American Nurses Credentialing Center).
A resume/CV:
• Include a resume/CV for the contact person and each instructor for the 3 required
course offerings submitted with this application:
• Please include individual faculty qualifications (Item #2) with resumes/CVs. (Submission
of a resume/CV is not required if you are accredited by the American Nurses
Credentialing Center).
• If applying as Board-approved and your organization is also ANCC approved please
attach a copy of your ANCC accreditation certificate.
• A RESUME contains factual and specific details of education, experience, job titles,
certifications, and skills. A BIO is an overview of the individual without any specific dates
or detailed duties. The BIO is used more for introducing the speaker or as a promotional
summary.
The following documents must be submitted ELECTRONICALLY for each of the three Course
Applications for review. Individual course documents may be attached after each course
submission.
• Submit the completed documents from the download file (found under course
application requirements) for each of the 3 courses submitted, including references in
APA format (for additional information regarding APA format please visit
http://www.apastyle.org/manual). All references should be within the past 5 years.
• A sample copy of your certificate: The Certificate of Attendance must include a place for
the following: Educational provider name, FL BOARD APPROVED & provider number,
course name, course tracking number, hours offered, completing date, participant’s
name, participant’s license number and the statement “Certificates must be retained for
at least 4 years, do not forward certificate to the Board of Nursing.”
• A resume/CV for the instructor submitted with the application. Please include individual
faculty qualifications with resumes/CVs.
Mandatory CE Program offerings:
• Three (3) initial original course offerings CANNOT be one of the mandatory CE
requirements or mandatory CNA in-services.
• Please see list of mandatory requirements listed in Florida Administrative Code 64B9-
5.002 Continuing Education Requirement below:
o (1) During each biennium, one contact hour must be earned for each calendar
month of the licensure cycle.
o (2) The following continuing education courses are a mandatory part of the
hours required in subsection (1), at the stated time periods:
▪ (a) A two-hour course in the prevention of medical errors each biennium;
▪ (b) A one hour course in HIV/AIDS in the first biennium only;
▪ (c) A two-hour course in Florida laws and rules each biennium;
▪ (d) A two-hour course in recognizing impairment in the workplace every
another biennium;
▪ (e) For biennial renewal on or after January 1, 2019, a two-hour course
on human trafficking, and each biennium thereafter;
▪ (f) A course in domestic violence every third biennium.
Specifically, on the application: I understand I cannot submit a Medical Errors, HIV/AIDs or
Domestic Violence course with this initial provider application.
All CNAs must complete a minimum of 24 hours of in-service training each biennium. In-service
training hours shall include, but are not limited to, the following areas:
• Bloodborne Pathogens, Infection Control
• Domestic Violence
• Medical Record Documentation and Legal Aspects Appropriate to Nursing Assistants
• Resident Rights
• Communication with cognitively impaired clients
• CPR skills
• Medical Error Prevention and Safety
Hours:
• All offerings shall be at least 60 minutes in length or one (1) contact hour.
• Increments of 30 minutes will be accepted when the offering extends beyond the one (1)
contact hour.
• Please review the number of hours submitted for each course. It should reflect the
number of contact hours that is on the objective form. (Example: Objectives indicates: 2
hours contact and 2 total clock hours. The number of contact hours awarded will be two
(2) hours usually under the general category. All hours should match.
• Identify all target audiences that will be receiving these credits. The target audience on
your objective form should match the hours that you have submitted for awarding to the
attendee. CNA’s are the only audience that does not receive continuing education hours.
Objectives:
• Learner objectives must be specific and measurable. Participants should be able to
demonstrate knowledge gained after course completion.
• Please review your objectives with Bloom’s taxonomy to help build critical thinking skills
within your course presentation. Describe, review, and discuss fall within the domain of
knowledge and comprehension (lower-level thinking skills) and does not build on the
more complex or higher-level thinking skills of apply, analyze, evaluate or create. Please
resubmit an objective form/course presentation that builds on the critical thinking skills.
Subject Matter:
• The content shall be specifically designed to meet the objectives and the stated level
and learning needs of the participants. The content shall be planned in logical order and
reflect input from experts in the subject matter. Appropriate subject matter for
continuing education offering shall reflect the professional educational needs for the
learner in order to meet the health care needs of the consumer and consist of content
from one or more of the following:
o Nursing practice areas and special health care problems.
o Biological, physical, behavioral, and social sciences.
o Legal aspects of health care.
o Management/administration of health care personnel and patient care.
o Teaching/learning process of health care personnel and patients.
o Personal development subject matter must include application of the content as it
relates to improved patient care.
• Subject matter must correspond to each objective. Information specific to an agency or
considered orientation or in-service training is not appropriate.
• The subject matter on the objective form should indicate what topics are being
discussed to meet the objectives and not just a repeat of the objective statement.

participant name, participant license number and the statement “Certificate must be
