Donald Church is 49 years old men, and he had a surge at the University of Washington Medical Center, and during his post-surgery time, he has had pain that was very hard to control. Donald had pain when he walked, site, bet over, and he has thought it was part of recovery. However, his pain stayed for around two months. The reason he had pain, when he had surgery, the surgeons accidentally left a 13-inch surgical instrument inside his body after an operation to remove a large malignant tumor in June 2000. Mr. Church awarded $97,000 in a settlement from the UW Medical Center. He apparently has not suffered permanent damage but still is filled with anger over the incident. MR Church was upset with what they did ad he stated “We’re not talking about leaving a wrench on somebody’s car engine. We’re talking about top surgeons at a major hospital doing this; this is somebody’s life”. (Warren King, 2001).

The Hippocratic oath is One of the oldest that write by Hippocrates. there are different versions of the Hippocratic oath. for instance, the classic version: “I swear by Apollo Physician and Asclepius and Hygieia and Panacea and all the gods and goddesses, making them my witnesses, that I will fulfill according to my ability and judgment this oath and this covenant” (William & Shiel, 2018). The modern version “I swear to fulfill, to the best of my ability and judgment, this covenant: I will respect the hard-won scientific gains of those physicians in whose steps I walk, and gladly share such knowledge as is mine with those who are to follow” (William & Shiel, 2018). I think the Hippocratic oath is a good way to promise not to hurt patients and to do the right job for patients.

nonmaleficence means “Actions intended not to harm or bring harm to other “(Sole et al., 2021 pg. 29). So, Maleficence means harming someone intentionally. Negligence is failing to do something carefully like a nurse not following the nurse’s scope of practice (Jacoby & Scruth, 2017). Who accepts the responsibility and liability for a medical error? I think It depends on the cause of the medical error. It could be the doctors, nurses, pharmacist, or it could be multiple parties. Whoever did the error is responsible also the hospital could be responsible based on how this error happened. To help prevent medical error is one of the ways is to follow QSEN. The QSEN six-core competence provides a foundation for quality care: patient-centered care, teamwork and collaboration, evidence-based practice, quality improvement, informatics, and safety. Applications of QSEN competencies to critical care nursing topics are integrated throughout this text” (Sole et al., 2021 pg. 5). Also, the Institute of Medicine of the National Academy of Sciences (IOM) also called the National Academy of Medicine helps to reduce mediation error. IOM has its own guidelines that will help everyone who is involved in patient care to prevent medication error.
