In my practical setting in a wound care clinic, the day started out, we looked at all the patients we needed to see for data collection and media review on what each patient needed. In the process, we discovered a wrong median was uploaded into a patient file, an armpit of a dark-skinned person into the file of a Caucasian patient file. This is a clear media error. How can we prevent this from happening, avoid distraction, making sure that one double check the camera before using it for proper identification?
